New Client Forms

These forms are for all new clients. Please read and complete them all, and bring them with you to your first appointment.

Aetna Form

This form is needed in addition to the new client forms if you are using Aetna Insurance. Please bring this form with the other new client forms to your first appointment. If you are not using Aetna insurance, then you do not need to complete this form.

Telehealth Consent Form

This form is for all new clients, as well.
